Once you complete the tutorial, and any time you reach an ending, you return to start a new game with a new daughter.
First you give your daughter a name.
Next you choose her birthday.
Your daughters starting stats will depend on her astrological sign.

Now choose your daughter's bloodtype.
Each one has a special effect at the end of each month,

Lastly you choose your daughter's future career. It doesn't guarantee that you will achieve that ending, but the game will help guide you towards it.

Once you've made all of your choices, you'll be presented with a summary screen to confirm.
Just click the bottom green button to accept and continue.
